Abstract
Tese de mestrado, Cuidados Paliativos, Universidade de Lisboa, Faculdade de Medicina, 2020 - Avaliação de necessidades em doentes oncológicos terminais acompanhados - Instituto Angolano de Controlo do Câncer e seus cuidadores, é um estudo que procurou avaliar as necessidades globais dos doentes oncológicos em fase terminal e seus cuidadores no I.A.C.C Instituto Angolano de Controlo de Câncer em Luanda-Angola, o único instituto oncológico no Pais. Mais especificamente, para realização desta pesquisa procedemos a 'priori' a um aprofundamento teórico e conceptual sobre a teoria das necessidades, indicadores dos níveis de necessidades humanas, mas relevantes para reflexão acerca do tema. Assim sendo, recorremos as contribuições de estudiosos do Serviço Social que analisam o cariz interventivo da profissão. Neste sentido foi dada a preferência a metodologia qualitativa, pois trata-se de um estudo de tipo descritivo. Amostra é constituída por a 34 sujeitos. Relativamente às técnicas para recolha dos dados recorremos aos questionários a entrevistas semiestruturadas realizadas, à 17 doentes e 17 cuidadores, (na sua maioria de sexo feminino totalizando 23 de sexo feminino e 11 de sexo masculino, com idades compreendidas entre os 20 anos e 72 anos), na sua maioria residentes em Luanda. Procurou-se compreender, identificar, e analisar de modo geral as principais necessidades exteriorizadas pelos doentes e seus cuidadores. Conclui-se que os impactos diretos ou indiretos da doença, têm incidência essencialmente nas necessidades básicas, primarias que os doentes e seus cuidadores vivenciaram ao longo do processo da doença. Atendendo aos custos com a medicação sugerimos também que tal situação seja revista e subsidiada na totalidade pelo Estado, principalmente fármacos à base de opiáceos para melhor controlo sintomático da dor. Os doentes e cuidadores evidenciaram claramente a importância caso fosse possível da integração de um assistente social nas equipas do IACC, as principais necessidades sentidas por parte dos cuidadores e doentes possivelmente poderiam ser atenuadas com maior facilidade, com a implementação de apoio ao nível dos transportes, alimentação, medicamentos, entre outros bens essenciais tão fundamentais para esta população. Esta investigação oferece um contributo importante, pois é primeiro trabalho a ser realizado em Angola com doentes em fase terminal. - Needs assessment in terminally ill cancer patients - Angolan Cancer Control Institute and its caregivers, is a study that sought to assess the global needs of terminally ill cancer patients and their caregivers at the I.A.C.C Angolan Cancer Control Institute in Luanda- Angola, the only cancer institute in the country. More specifically, in order to carry out this research we have "priori" to a theoretical and conceptual deepening on the theory of needs, indicators of the levels of human needs most relevant for reflection on the subject. Thus, we have resorted to the contributions of Social Service scholars who analyze the intervening nature of the profession. In this sense, qualitative methodology was given preference, as it is a descriptive study. The sample consists of 34 subjects. Regarding the techniques to collect the data, we used questionnaires and semi-structured interviews carried out with 17 patients and 17 carers (most of them female, 23 female and 11 male, aged between 20 and 72 years), most of them living in Luanda. We tried to understand, identify, and analyse in general the main needs expressed by patients and their carers. It was concluded that the direct or indirect impacts of the disease have a focus on the basic, primary needs that the patients and their caregivers experienced throughout the disease process. Given the costs of medication we also suggest that this situation should be reviewed and fully subsidized by the state, mainly opiates-based drugs for better symptomatic pain control. Patients and caregivers clearly highlighted the importance of integrating a social worker into the IACC teams, the main needs felt by caregivers and patients could be more easily alleviated, with the implementation of transport, food, medication, among other essential goods so fundamental to this population. This research offers an important contribution, as it is the first work to be carried out in Angola with terminally ill patients |
